Please do not submit Social Security numbers, insurance policy numbers, or other sensitive documents through this form.

How it is shared

• Form submissions are stored in the firm's client intake system, operated by our customer relationship management provider (HighLevel), which processes the information on our behalf.
• This page uses the Meta Pixel to measure visits and completed requests from Facebook and Instagram ads. Meta receives page-visit information and a notice that a request was submitted. The contents of your form, including your claim status and details, are not sent to Meta.
• We do not sell your personal information. We may disclose information when required by law.
